Getting started

Contact us by phone at 406.451.7370 if you have any questions.

If you are ready to refer a patient to us, fax us a face sheet with your most recent visit note to 406.571.7956. We will then reach out to the referred patient to schedule an initial call.

When a new patient enters our program, we begin with a thorough 3 step assessment process.

Step One

An initial screening telephone call.
Common barriers to entering treatment are often addressed at this time.

Step Two

Verify health insurance coverage (if the patient desires).

Step Three

Intake assessments with an EDCMT:
  • therapist/intake specialist.
  • registered dietitian.
  • psychiatrist (depending on severity of the patient’s symptoms)

Determining the Level of Care

The intake assessment underpins our recommendation for treatment level of care. Your patient can then make an informed decision about the recommended program level of care.

During the initial assessment, patients complete several intake forms. This includes a release of medical information form, allowing us to update you on their progress through our treatment program.
